Skin Tears are wounds of traumatic etiology, occurring mainly in the extremities of the elderly population. These result from friction, or frictional forces of joint action and shear that separates the epidermis from the dermis, or both that separate them from the underlying layers. In the evaluation of wounds, appropriate tools enable a standardized evaluation, measure and reflect valuable information in evidence-based decision making to health professionals. Although there is no universally accepted instrument for the assessment of skin breaks, its existence is important to assess the wound and help plan the care as it is a growing problem with a negative impact on our society and often ignored. The objectives of this dissertation are: Perform cultural and linguistic adaptation into Portuguese in Portugal STAR Skin Tear Classification System and assess their degree of reproducibility; characterize the population of nurses who assisted the validation of the instrument for service time and formation in wounds and tissue viability and to identify the clinical characteristics of patients with skin breaks and its etiology. This study is relatively of quantitative methodology and of data analysis. The STAR Skin Tear Classification System is a tool for diagnosis and treatment of skin breaks, consisting of a guide to treatment, a rating system, which has five categories and finally a glossary with definitions and technical terms related to breaks in the skin care and peri-lesional skin. Being a complete instrument and accepted internationally proceeded to the translation and validation of the same instrument with a grade of 97% reproducibility in the implementation of photos and 76% in clinical validation, proving to be a reliable tool which can be used by other researchers.
