Abstract

Under the Master in Nursing with specialization in Community Nursing at the Health Sciences Institute of Portuguese Catholic University and as a consequence of the stage held between April 27, 2011 and February 4, 2012, in the Community Care Unit of Senhora da Hora, was elaborated this report, which shows the route taken over the three training modules, intending to demonstrate the skills acquired to the degree of Master in Nursing and still be an opportunity to reflect on all the work and recognition of what was done in face of unforeseen situations and obstacles encountered. The methodology used during the stage and this report was the Health Planning, which goes through several stages, from diagnosis to the health assessment of the implementation of community intervention projects. In the module I stage was conducted health diagnosis of the target population: diabetic users of private clinics and residents in the village of Senhora da Hora, identifying the main health needs, of which could be highlighted the inadequate monitoring of blood glucose monitoring, the non adherence to treatment regimen (diet, exercise and medication regimen), the health seeking behavior compromised, the inadequate monitoring of the feet and the use of community resources and sources of information ineffective. After setting the priorities, it was prepared the community intervention project "Watch your diabetes ... Take care of yourself," which focused on global health surveillance. Relevant results were obtained, with 96% of users present in the AEPS acquiring knowledge on monitoring of blood glucose monitoring, 68% of those called to tetanus vaccination took at least one dose and 26% of users called for a Nursing consultation attended. Modules II and III were intended to project implementation and its evaluation as well as the understanding of other experiences that have enabled a broader view on the role of nursing in primary health care.
