Abstract

The present report is to present the activities carried out during the stage, showing how were acquired skills that allow the Specialization in Community Health Nursing. The stage was held in UCC Senhora da Hora, in the periods between April 27th and June 25th, 2011 and between October 3rd, 2011 and February 4th, 2012, in three modules. The methodology of health planning was used. During the first Module was planned, executed and presented a Diagnosis of Health Situation, with the target population composed by the Diabetic clients of Private Clinics and residents in Senhora da Hora. A form was applied to a sample of 63 individuals, out of 190 people. The aim was to understand the problems/needs of this population and then was made to Priority Setting. In Modules II and III was planned, implemented and evaluated an Intervention Project to tackle the problem: inadequate surveillance and care of the feet. The design theme was: "Vigilance and Care for the Diabetic Foot" and the main objective was to induce behavior change in preventive health and seeks to the target population, contributing to reducing the appearance of foot ulcers on diabetic foot. The key strategies were: health education, home visitations and a Health Fair to commemorate the World Diabetes Day. Education sessions for health have increased knowledge of clients and their families/caregivers about hygiene, surveillance and care for feet, with a percentage of 84% correct responses showed at the end of sessions and reaching 40% attendance, of all clients called. The home visitations were made to assess the condition of the feet of clients and to evaluate their family. This intervention attained 36,3% of the target population in regard to the evaluation of the feet, helping to achieve the indication given by DGS that each diabetic must have at least one annual evaluation of his foot. All records in the clinical process of clients were made.
